Your survey is pretty poorly written. Your first real question asks how often the respondent drinks "soft drinks", without defining the term. Do you mean soda, or all non-alcoholic beverages?
Well it's not really nice to say it's poorly written now is it Ellis? I thought it was fabulous and most (American) people know that when you refer to a soft drink, it means a soda, at least in my neck of the woods.
May not be "nice", but it's accurate. If this survey is actually for some kind of research project, then terms need to be precisely defined for the resultant data to be meaningful. Well, here's a definition of the term, according to: http://www.wordiq.com/definition/Soft_drink Lemonade, Iced Tea, Fruit Juices, Seltzer, and even bottled water ALL fit the definition, not just soda.
