A friend of mine made something a lot like that with a beaker, an old fashioned rubber tub-stop and some rubber tubing from the hardware store. For the bowl I think he just bought one at a local store called Lifestyles. I can't help much with the construction as I got to use it long after it was made, but there's some ideas to help you out.
Dude easy. For the mouth piece just use some rubber or plastic. For the stem you can use the same or a hollow metal rod. For the bowl you can use Tinfoil, make one out of wood, buy one, or just use a large metal screen and fashion it into a bowl.
