    1. Nope, not that happy with my daily eating tend to consume lots of crappy food I know I shouldn't

2.I'd like to eat more organiclly and I've also toyed with becoming a vegetarian for quite awhile.

    3. I have little control over how I eat because I live with my mom and what she buys I eat for the most part. I also eat lots of pizza and burgers and stuff because I'm to lazy to make food much of the time and to me these seem easy

    4. My mom is not very good role model for my eating. In many ways, she eats worse than me because I don't drink soda and eat more vegies than her.

    5. I am not positive though they may have offered to take me to eat somewhere extremely unhealthy if I raked the leaves or something. I can't say for certain, but pit probably happened

    6. No, I was free to eat as I liked. I ate anything I wanted and we had lots at are house.

    7. Well, now that you mention it I did eat lots of potato chips on vacationbecause we rarely had them at home. Once, I even vomited from a potato chip binge this was at age 5 or so. But I don't think it was related to them being there or not just avalibility.

8.Potato chips are good on occasion but not an ever day thing.

    9. I plan on becoming a vegetarian and eat more organic foods, though I don't know when

10.No, its easy to change what you eat when you change your life style giving up msoda was simple once I replaced it with juice and I think I can easily begin to eat organic food through the elmination of the non-organic products in my house.

    11. I think my mom did her best. She is a single mom working he rhardest to get us through college and beyond. Eating habits were not on the top of her priorities list and the way we ate was subsequently not amazing but she tried.

    13. Is it similar tothe US food guide? If so no not at all. If its based on a 2000 calorie diet I dont follow it in the least I eat way more than that. If it talks about how many grains, vegies, meats, fruits, etc. you must eat I also pay no attention to it.

    14. I'd like to say I have more important things to feel guilty about but I do sometimes think" Man, I should order that grilled chicken salad but the bacon bleu chees burger looks sooooo good" Then, feel like I should have gotten the slad afterwards. It doesnt stop me from enjoying my burger though.

    15. Yes, I used to be on a crew team and work out 3 hours a day but I quit cause Im lazy and I currently go to the gym 4 days a week.

    17. The way our culture and media portray beauty is really troubling and I think the west needs a cultural revolution in many ways. Body image shoiuldn't caus esuch suffering. And though eating disorders are part mentality part Society, we could stop many cases of eating disorder with some change in our idea of beauty.

    19. Yes, health class had spent countlaess hours drilling these facts into my head. But I'm not concerned by them too much.

    20. If I continue to eat exactly as I do, then maybe I will see some problems in the future but I think if I make some adjustments give up fast food and such I can extend my life span.

    21. Yes, I will change soon..... waits

    22. 5, not awful not great kinda meh. I plan on being more concerned with my eating habits tomarrow or the next day. Haha, no I will actually change the way I eat soon.

PS. Im sorry for the probably countless gramatical errors in my survey I took it with haste becuase I have to study for finals.

Food & Nutrition Survey - Eating Habits

Are you happy with the way you eat daily?

Only a few days of the week, am I actually happy about it.

If not, then do you want to improve? Explain.

I waver between diets, and I'd like to stick to one, and actually follow it for the most part.

Do you have good control over the way you eat?

I tend to binge eat or eat very little, so, no.

As a child, do you think your parents were good role models when it came to eating? Explain.

My parents never withheld any foods from us as kids, neither with the amount or type of food. We ate what I think was generally a good, healthy diet, at least. So, yes, I think they were good role models.

Did your parents ever use food as a type of a reward or punishment?

    Not at all.

Did your parents restrict you from certain types of food?

    No.

Did you find yourself eating more of that food when they weren’t around?

    No, as I wasn't restricted.

If so, how do you feel about eating those foods?

Do you want to change the way you eat?

Yes. I'd like more consistancy in the way that I eat.

Do you find it difficult to change from the way you ate when you were younger until now? Do you still eat the same way?

No, things changed gradually, and I did not notice it. I definitely do not eat the same way as I used to as a child.

Do you think your parents did a good job practicing healthy eating habits? Why or why not?

I was a very picky eater as a child, as far as what I genuinely did and did not like to eat, tastewise. My parents catered to that a lot, and I'm not entirely sure that I was getting everything I needed nutritionally. I was never forced to eat anything I didn't want to. Maybe this isn't good on their behalf? My parents, themselves, actually did practise healthy eating habits, however. Lots of good, fresh foods all the time, with other things in moderation.

Who would you say influenced your eating habits the most? Check all that apply.

    □ Friends

Do you follow Canada’s food guide to healthy eating? (Amount of food you require daily, from the four food groups)

Not anymore, no. Far from it.

How often do you feel guilty about eating?

    About 80% of the time.

    Do you Exercise?

Yes, I bicycle or walk for at least an hour every day, and work out roughly the same amount of time.

Do you suffer from any of the eating disorders? Check all that apply.

    □ Compulsive Eating Disorder
    □ Anorexia
    □ Obesity
    □ Bulimia - Not as bad as it has been in the past, but it still does linger quite a bit.
    □ None

How do you feel about that? Explain.

I'm tired of it. I'm sick of the guilt, the pressure I put on myself, constantly overtired, headaches, I always feel inadequate, even though I'm at a very healthy range for my height and structure.

Does your family have a history of any of the following due to their eating habits?

    □ Heart Disease
    □ High Blood Pressure... And type II diabetes, though it isn't listed.
    □ Kidney Failure
    □ Stroke

    □ Bone and Joint Problems
    □ Psychological Problems (depression, lack of self esteem)
    □ None
    □ I don’t know

Are you aware of how bad eating habits can lead to these problems?

    Yes.

Do you think your eating habits can lead to any of these problems?

Definitely, especially psychological, amongst various others that I've already experienced.

If so, are you willing to change in order to prevent these from happening?

    I really would like to.

From a scale of 1 - 10, rate your eating habits. (1 being poor, 10 being excellent). Comment

Lately, maybe between a 1 and 3. I know that I still do not get all the nutrients that I need, I either under-eat or over-eat (and then compensate for it with under-eating or something else), Or, I just find myself eating the wrong kinds of things, and know I do not get the proper nutrients, or enough to sustain myself physically. Other days, I do eat a good, balanced diet, and get exercise, but I don't always feel good about it.
