Many people here are curious about things like "How much Oxy is 40mg of Opana ER equal to". These can be dangerous situations if you miscalculate the strengths. On the other hand, if you calculate too low, you may not get any effects. I have an App that is used by doctors for converting opioids. I will post the conversion factors below: We will use 30mg of Morphine, Oral, as the standard 30mg Morphine Oral =0.3mg Buprenorphine IV =1mg Suboxone Sublingual (this was not from the app but personal experience) =200mg Codeine Oral =10mg Heroin IV =0.3-0.45mg Fentanyl Transdermal/Buccal =30mg Hydrocodone Oral =7.5mg Hydromorphone Oral =1.5mg Hydromorphone IV =300mg Meperidine Oral =2-20mg Methadone Oral (note the range in values) =20mg Oxycodone Oral =5-7.5mg Oxymorphone Oral (adjusted based on other data) =130-200mg Propoxyphene Oral =250mg Tramadol (based on other data) This isn't perfect, and there is NO one chart that has all of the values correct. But this is altered to meet the most realistic information I could find. Use this wisely and always take the lower bound to convert so you don't take too much. In fact, doctors recommend starting a new opioid at 70% of the true equivalent dose of the old opioid. Take that as a good warning!
