It's a real condition, but by no means as common as it's made out to be. It's caused by blood flow being restricted to the testes, but is often mistaken for the more common condition where the erection can be exceptionally hard for so long without relief that it becomes painful. This doesn't even require any sexual activity - even uncontrolled Nocturnal Erections (even when on one's own) can cause it.
