I would be really careful buying something like this off of Ebay. There are other reputable sources out there. To answer your question, you can smoke the leaves or extract.
The bags of salvia I buy from the headshop have a sticker that says "for use as incense" on them. It's probably just a legal thing.
Yep I just bought a 1/4 lb from the salvia king (the ebay seller), and it got here just great no problems at all
