Popped my answers in too - Seems doing maths in front of a crowd is more of a problem than the math itself... Not to spoil any suprises or owt.
answered as best i could, but some of the questions were tricky, my reaction would depend on entirely on the context. for example, how important doing a piece of maths actually was, time limit, etc.
Really weird survey. Are you trying to determine if peoples unwillingness to math is caused by anxienty, thus mental condition and therefore it can be cured?

guess i'm not so anxious as I thought! I'm not from the UK so i had no idea what my math level would be. Probably shouldn't have clicked Bachelors Degree
