    this survey isnt very intuitive. You should either give descriptions of the less obvious ones or make a selection for I don't know what the hell that is. also, your catagories are dumb. they should be something along the lines of "I do it", "I don't do it but would like to", "I don't do it and would never", and "I don't know what that is".

    i didn't fill out your survey for these reasons
