I see what you did there. The difference is that america is usually repsonsible for these things, whereas they do have a habit of inadvertently killing innocent civilians along the way. Statistically, the assumption that it was the US is more likely to be true, by a large margin. It's all for a greater cause though, democracy. Because unless we have democracy there is no way we are ever going to tackle the REAL issues facing the planet.

There has been a general trend in war in recent years, the number of civilian casualties in war has risen dramatically, and by contrast the number of military deaths has dropped. This is a result to technological developments, coupled with radical changes in military strategy. In the old days there were more front line battle fields, now countries at war, particularly the US and it's western allies, prefer to coerce nations into surrendering by bombing them, often at great cost to civilian lives. The Iraq war is a terrible example of this, and Vietnam.

No, the US and Allies would prefer to fight the enemy on the open battlefield"which would be utter suicide for them", it's the insurgents that have made it an urban war. The majority of Jihadist elements use car bombs, kidnappings, hostage-taking, shootings and other types of attacks to target Iraqis and U.S. forces with little regard for civilian casualties.
I think civillian deaths have decreased dramatically. It is difficult because you can't compare the Iraq war with any previous war. It isn't like Vietnam...it certainly isn't like any of the world wars. Perhaps civillian deaths are used in the way you use them, more. They are thrown at people to indicate a mentality.
